As a nurse and as a mother navigating disability and care fragmentation, I’ve seen how quickly people fall through the cracks—not because they don’t care, but because the system is complex, disconnected, and hard to navigate. Health literacy varies. Community resources are uneven. And “innovation” often shows up without the lived context that determines whether it works.

• Start with the patient: their goals, realities, barriers, and strengths. 
• Build trust first: relationship-based care is the infrastructure. 
• Let patients shape the narrative: care is built with people, not for them. 
• Connect clinical care to real life: home, transportation, food access, caregiving load, finances, understanding.
• Make support scalable: capture real-time needs and match to existing community resources in a repeatable way.
